The Patient Advocate serves as a liaison between patients and ELFHCC to ensure that patients receive the care, support, and services they need. This role is critical in addressing patient concerns, improving the patient experience, and ensuring that the health center operates in a patient-centered manner. The Patient Advocate will work closely with medical, administrative, and support staff to resolve issues, provide education, and advocate for patient needs, ensuring compliance with ELFHCC standards and regulations.

Qualifications:

  • High school diploma or equivalent required; Associate's degree or higher preferred.
  • Formalized training or certification in enrolling patients into managed care plans is a plus.
  • Bi-lingual in English/Spanish or English/Portuguese is a strong plus.
  • Previous experience in a Community Health Care Center or primary care office preferred.
  • Must possess the ability to interact respectfully and effectively with diverse cultural and socio-economic populations and shareholders.
  • Must have a valid PA driver's license and access to a reliable personal vehicle. Local travel between sites is required.

Key Job Responsibilities:

    • Maintains established organizational policies and procedures, objectives, performance improvement program, safety, environment of care, management of information, and infection control standards.
    • Utilizes excellent customer service and team skills at all times.
    • Complies with federal and state law, regulations, accrediting, and licensing agencies at all times.
    • Provide enrollment assistance for uninsured children and adults to access subsidized, low-cost, and free health insurance programs through the health insurance marketplace, Medicaid, and the Children's Health Insurance Program (CHIP).
    • Provide structured patient education on health coverage, engage in follow-up conversations, and offer renewal assistance for enrolled individuals.
    • Distribute outreach materials to patients, community members, partner organizations, and businesses to build coverage option awareness. Leads presentations at local community events/organizations.
    • Develop, discover, and attend community events in order to promote coverage options and the mission and services of the organization.
    • Collaborate with various local organizations to build awareness of coverage options, spur enrollment and build referral linkages.
    • Accurately provide required reporting to track goal achievement and client satisfaction.
    • Assist in the development and implementation of organizational outreach and enrollment initiatives.
    • Attends and participates in meetings and in-services as required and/or assigned.
    • Must have strong computer skills, including proficiency with the Microsoft Office suite, email, and internet enrollment platforms.

Physical Demands: Work is performed in a pleasant and community setting with local travel required. 60% of duties will be performed in-office and 40% of duties will be in-field performing outreach and enrollment assistance functions. Physical demands include prolonged sitting/standing and phone use. Lifting and carrying PR equipment and supplies (5-10 lbs.) may be required. Reaching, stooping, bending, kneeling and crouching may apply in various situations, and visual acuity is required to perform tasks.

Compensation: Salary commensurate with education and experience.
