The Department of Family Medicine and Community health has an exciting opportunity for a part time Health Coach position. The incumbent partners with patients to address health and wellness goals that are motivating and personally meaningful. The coach establishes a supportive, collaborative relationship, works with the patient to set meaningful goals and design and support small action steps toward those goals. The coach uses an evidence-based process that facilities sustainable behavior changes while fostering patient autonomy and self-discovery. Coaching may occur individually and in small groups, and in a variety of formats: virtually, telephonically, and/or face-to-face.
CORE JOB FUNCTIONS - Develop trust and rapport through mindful listening and other-focused communication.
- Establish a safe, values-based, and strengths-focused relationship.
- Promote patient autonomy and individual responsibility.
- Employ evidence-based behavior change skills to help patient explore their values and optimal health vision.
- Assess patient readiness and willingness to change
- Elicit patients' goals to improve health and overall wellbeing
- Connect patients' values and vision to long-term goals
- Support individual action steps with tracking, accountability, and consideration of potential barriers and backup plans
- Empower clients to self-discover behavior change strategies for ongoing success

- Health Coach training through an approved course by NBHWC
- Board certification provided by the National Board for Health & Wellness Coaching (NBHWC)
- Minimum 2 years of direct patient experience
